Individually, freely and voluntarily, and in accordance with the provisions of articles 27, 28, 29, 30, 31 and 32 of the statutes of the National College of Democrats (CND), as well as the regulations aimed at standardizing the integration and functioning of the National College of Democrats (CND) political party, I declare that I fully exercise my fundamental rights as a Haitian citizen enjoying his constitutional rights. Thus, I present myself before this party body to request my membership in the National College of Democrats, in respect of the terms and procedures provided for by the regulations in force. To this end, I have attached to this document the annexed documents constituting my respective file. Under protest of telling the truth and the whole truth, I affirm that I have never been, nor am I currently, a member of another political party as long as I belong to the National College of Democrats. Holding a position within the organizational structure of the party, I undertake to preserve the confidentiality of all data and essential information discussed internally.
